Non-Discrimination and Compliance with Section 1557 of the Affordable Care Act

Panhandle Home Health, Inc. does not discriminate on the basis of race, color, national origin, sex, age or disability in any home health program or activity.

Panhandle Home Health, Inc. does not discriminate based on and individual’s sex, related medical conditions, including pregnancy or termination of pregnancy, gender identity or sex stereotypes.

Instructions for filing a grievance are contained in the patient folder and may also be found on the website at www.panhandlehomehealth.org.

You may file a civil rights complaint with OCR if you feel a health care provider or government agency discriminated against you (or someone else) unlawfully.

If you believe that you have been discriminated against because of your race, color, national origin, disability, age, sex (including sex stereotyping and gender identity), or religion in any home health program or activity, you may file a complaint with Office of Civil Rights (OCR). You may file a complaint for yourself or for someone else.
