Physical Therapist [ independent contracts are available]

PRIMARY FUNCTION

To furnish physical therapy services in accordance with the plan of care in the patient’s home.

POSITION SCOPE

To deliver comprehensive physical therapy services to patients in their homes,

To ensure the provision of safe, quality, and cost effective physical therapy services, and

To provide physical therapy services in a manner which conveys the philosophy, mission, vision, and goals of Panhandle Home Health, Inc. (PHHI).

PERFORMANCE RESPONSIBILITIES

  1. Behaviors are consistent with the philosophy, mission, vision, and goals of PHHI,
  2. Works as a team member with all PHHI departments, supervisors, and coworkers,
  3. Demonstrates critical thinking and decision making abilities in assessment, treatment           planning, physical therapy services, and evaluation,
  4. Documents on all required forms and ensures confidentiality of all patient and family          information,
  5. Takes responsibility for identifying learning needs and acquiring updated practice    knowledge to enhance professional growth,
  6. Participates in performance improvement (PI) activities,
  7. Provides coordination, referral, consultation, and education,
  8. Provides direction, supervision, and evaluation of Physical Therapy Assistants,
  9. Assigns and supervises the Certified Home Health Aide,
  10. Adheres to all PHHI policies, procedures, and regulations,
  11. Adheres to the Corporate Compliance Plan,
  12. Participates in all required meetings/inservices/committees as scheduled,
  13. Performs other duties as assigned, and
  14. Demonstrates proficiency in all technical competency skills as customarily expected for       licensed Physical Therapist.

POSITION QUALIFICATIONS:

  1. A current license, in good standing, to practice as a Physical Therapist in West Virginia,
  2. A current certification in basic Cardiopulmonary Resuscitation (CPR),
  3. Experience as a Physical Therapist,
  4. Able to utilize independent critical thinking and decision making skills,
  5. Exercises good judgment,        
  6. Possesses good interpersonal communication skills,
  7. Able to get along with people,
  8. Proficient in technical skills,
  9. Does not pose a direct threat to the health and safety of self or others in the workplace, and
  10. Has not been convicted of a crime or suspended or excluded from participation in Medicare / Medicaid Programs or other federal health care programs.

REPORTS TO: Clinical Manager

SUPERVISES: Physical Therapy Assistant

OSHA BLOODBORNE PATHOGENS CLASSIFICATION: Category I

ESSENTIAL FUNCTIONS :

  1. Provides all physical therapy services,
  2. Assists the physician in evaluating level of function,
  3. Helps develop the plan of care (revising it as necessary),
  4. Prepares clinical and program notes,
  5. Advises and consults with the family and other agency personnel,
  6. Participates in inservice programs,
  7. Assign and supervise the Physical Therapy Assistant according to the West Virginia Board of Physical Therapy standards,
  8. Assessment and discharge of all Physical Therapy Assistant patients,
  9. Assigns, trains, and supervises Certified Home Health Aide, and
  10. Complies with accepted professional standards and principles of the West Virginia State Practice Act for the provision of Physical Therapy.

Certified Home Health Aide

PRIMARY FUNCTION

To furnish patient care as assigned by a registered nurse (RN), speech therapist (ST), physical therapist (PT), and/or occupational therapist (OT).

POSITION SCOPE

To provide patient care supervised by a RN, ST, PT, and/or OT to patients in their homes, and

To provide patient care in a manner which conveys the philosophy, mission, vision and goals of Panhandle Home Health, Inc. (PHHI).

PERFORMANCE RESPONSIBILITIES

  1. Behaviors are consistent with the philosophy, mission, vision, and goals of PHHI,
  2. Works as a team member with all PHHI departments, supervisors, and coworkers,
  3. Performs patient care as assigned,
  4. Documents on required forms and ensures the confidentiality of all medical records,
  5. Attends and participates in inservice training no less than twelve (12) hours each calendar year (prorated if less than one calendar year of employment),
  6. Participates in performance improvement (PI) activities,
  7. Adheres to all PHHI policies, procedures and regulations,
  8. Adheres to the Corporate Compliance Plan,
  9. Participates in required meetings, inservices, and committees as scheduled,
  10. Performs other duties as assigned, and
  11. Demonstrates satisfactory completion of a competency evaluation at the end of orientation and technical skills performance on an annual basis.

POSITION QUALIFICATIONS:

  1. High School Diploma or equivalency,
  2. Satisfactory completion of competency evaluation,
  3. Possesses current certification in Cardiopulmonary Resuscitation (CPR) at basic life       support level,
  4. Sympathetic attitude toward the care of the sick,
  5. Maturity,
  6. Ability to deal effectively with the demands of the position, 
  7. Exercises good judgement,
  8. Possesses good interpersonal communication skills,
  9. Able to get along with people,
  10. Does not pose a direct threat to the health and safety of self or others in the workplace, and

OSHA BLOODBORNE PATHOGENS CLASSIFICATION: Category I

ESSENTIAL FUNCTIONS :

  1. Performs simple procedures as an extension of therapy services,
  2. Performs personal care,
  3. Performs patient ambulation and exercise,
  4. Performs household services essential to health care at home,
  5. Assists with medications that are ordinarily self-administered, 
  6. Reports changes in the patient’s condition and needs,
  7. Completes appropriate records,
  8. Maintains home health aide certification.

Registered Dietitian

To provide consultative and nutrition services in a manner which conveys the philosophy, mission, vision, and goals of Panhandle Home Health, Inc. (PHHI)

To ensure the provision of safe, quality, and cost effective skilled nursing services, and

To provide patient care in a manner which conveys the philosophy, mission, vision and goals of Panhandle Home Health, Inc. (PHHI).

PERFORMANCE RESPONSIBILITIES

  2. Works as a team member with all PHHI departments, supervisors, and coworkers,

  3. Available, as needed, for twenty-four hour coverage,
  4. Demonstrates critical thinking and decision making skills, 
  5. Exercises good judgment,
  6. Possesses good interpersonal communication skills,
  7. Able to get along with people,
  8. Proficient in technical skills,
  9. Does not pose a direct threat to the health and safety of self or others in the workplace, and
  10. Has not been convicted of a crime or suspended or excluded from participation in Medicare / Medicaid Programs or other federal health care programs.

ESSENTIAL FUNCTIONS :

  1. Makes the initial assessment visit and regularly re-evaluates the patient’s nursing needs,
  2. Initiates the plan of care and necessary revisions,
  3. Provides those services requiring substantial and specialized nursing skill,
  4. Initiates appropriate preventive and rehabilitative nursing procedures,
  5. Prepares clinical and program notes, 
  6. Coordinates services,
  7. Informs physicians and other personnel of changes in the patient’s condition and needs,  
  8. Counsels the patient and family in meeting nursing and related needs,
  9. Participates in inservice programs,
  10. Supervises and teaches other nursing personnel, and
  11. Complies with accepted professional standards and principles of the West Virginia State     Practice Act for the provision of Registered Professional Nurses.
